Acne on the back, shoulders and chest is common, often more stubborn than facial acne, and very easy to ignore until wedding season or a beach holiday comes around. The skin on the back has many oil glands, is covered by clothing all day and gets sweaty, which creates the perfect conditions for breakouts.
Here is what causes it, the back acne treatment routine dermatologists recommend, how to tell if it is actually fungal acne, and what helps clear the dark marks it leaves behind.
What causes back acne?
Back acne forms the same way facial acne does: oil, dead skin cells and acne bacteria block hair follicles and trigger inflammation. Several things make the back especially prone:
- Lots of oil glands on the upper back and chest
- Sweat and heat, especially in Delhi's humid months or after workouts
- Friction and pressure from backpacks, bag straps, tight gym wear and car seats
- Hormones, including during the teens, around periods and with conditions such as PCOS
- Hair and body products – heavy conditioners and oils running down the back in the shower
- Genetics – body acne often runs in families
- Certain supplements and medicines – some people notice breakouts with anabolic steroids or high-dose B12; a link with whey protein has been reported in some cases. Never stop a prescribed medicine without talking to your doctor.
Is it acne or fungal acne?
Not every bump on the back is acne. Malassezia (pityrosporum) folliculitis, often called fungal acne, is caused by an overgrowth of yeast in hair follicles. It is common in hot, humid weather and after sweating.
| Acne | Fungal acne | |
|---|---|---|
| Look | Mix of blackheads, whiteheads, red bumps, sometimes deep cysts | Many small, uniform bumps of the same size |
| Itch | Usually not very itchy | Often itchy |
| Location | Back, chest, shoulders, face | Upper back, chest, shoulders |
| Usual treatment | Benzoyl peroxide, retinoids, antibiotics if needed | Antifungal washes or medicine |
Fungal acne does not respond to standard acne products and can be worsened by antibiotics, so a correct diagnosis matters. Our guide to fungal skin infections explains more.
A dermatologist-approved routine for back acne
The American Academy of Dermatology suggests a simple approach for clearer skin on the back:
- Use a benzoyl peroxide wash in the shower. Apply a foaming wash to the back and leave it on for 2–5 minutes before rinsing so it has time to work. Start with a lower strength around 5% to limit irritation. Benzoyl peroxide bleaches fabric, so use white towels and old T-shirts.
- Add adapalene 0.1% gel. Applied daily after showering or at bedtime, it helps unclog pores so the benzoyl peroxide works better. Expect some dryness at first.
- Wear loose, breathable clothes. Choose cotton or sweat-wicking fabrics, and wash workout clothes after every use.
- Shower soon after sweating. If you cannot, wipe your skin with an oil-free cleansing wipe and change into dry clothes.
- Reduce friction. Swap a heavy backpack for a handheld bag where possible.
- Protect exposed skin from the sun with a non-comedogenic SPF 30+ sunscreen, because sun exposure can darken acne marks.
The AAD says results may appear in 6–8 weeks, and complete clearing can take 3–4 months. Consistency matters more than strength.
Extra habits that help
- Rinse conditioner and hair oil off your back before washing your body
- Change bedsheets and pillowcases weekly
- Do not scrub hard with loofahs; friction makes inflammation worse
- Do not pick or squeeze, which causes dark marks and scars
When to see a dermatologist
The AAD recommends seeing a dermatologist if your acne is deep or painful, or if you see no difference after 6–8 weeks. Also book a visit if:
- The bumps are very itchy or all look the same (possible fungal acne)
- You are getting scars or many dark marks
- Acne started suddenly in adulthood or comes with irregular periods or excess hair growth
- It affects your confidence or what you wear
A doctor can prescribe stronger topical treatments, oral antibiotics for a limited period, hormonal treatment in suitable women, or oral isotretinoin for severe or scarring acne under close supervision.
Clinic treatments for stubborn back acne
Alongside prescribed medicines, in-clinic treatments can speed things up:
- Chemical peels for the back. Salicylic or glycolic acid peels unclog pores, calm breakouts and lift dark marks. See our glycolic peel page.
- Body polishing with a peel. A clinical exfoliation and brightening treatment that suits congested, uneven skin on the back. See body polishing with peel.
- Acne laser. Laser and light treatments can calm active, inflamed acne. Read about our acne laser treatment and whether laser can treat active acne.
How to clear back acne marks
Once breakouts are under control, dark marks usually fade, but on the back they can take longer because the skin is thicker and often overlooked. To speed things up:
- Keep using sun protection on any exposed areas, such as with backless blouses
- Use fading ingredients such as azelaic acid, glycolic acid or retinoids, as advised
- Consider a course of peels or body polishing with peel
- For stubborn brown marks, a doctor may suggest laser toning in suitable cases
Our guide on acne marks vs acne scars explains how to tell flat marks from scars and how long they take to fade. Textured scars on the back need different treatment.
What does it cost?
The cost depends on what your doctor prescribes and whether you need in-clinic treatment. As a guide, body polishing with peel at Paradise Wellness is ₹7,999 per session or ₹28,999 for a package. Your exact plan and price are confirmed at consultation.
Frequently asked questions
Why do I get acne on my back but not my face?
The back has many oil glands and is exposed to sweat, friction and heavy products, so it can break out even when your face is clear.
Does whey protein cause back acne?
Some people notice more breakouts with whey, and a link has been reported, but it does not affect everyone. If you suspect it, discuss it with your doctor before cutting it out.
Can I wear a backless blouse with back acne?
Yes, and treatment before an event helps. Start a routine at least 2–3 months before, and avoid trying new strong products right before the day.
Is back acne a sign of a hormone problem?
Not usually, but adult-onset acne with irregular periods or extra hair growth should be checked.
Will back acne go away on its own?
Mild back acne may improve with time, but most people clear faster and with fewer marks when they treat it properly.
The bottom line
Back acne comes from the same process as facial acne, made worse by sweat, friction and products. A benzoyl peroxide wash with proper contact time, adapalene, breathable clothes and quick showers after exercise clear many cases in a few months. If bumps are itchy and uniform, think fungal acne. For stubborn breakouts or marks, a dermatologist can add prescription treatments, peels or laser. Learn more on our acne and breakouts page.
